Surgical Guide Design Software: Running a Design Service
For Small Dental Labs
This cookbook is written for small dental labs that already have some form of digital workflow and want to launch or standardize a surgical guide design service. It is practical, step-by-step, and based on how GuideMia dental implant software and the Cympha platform are actually used in production.
This document focuses only on implant surgical guides. A separate cookbook is available for clear aligners.
1. What you need to start a surgical guide design service
To operate a surgical guide design service, you need five fundamental components: software, collaboration, case management, manufacturing, and training/support.
A) Surgical guide design software (lab side)
GuideMia Implant Master — sold in three tiers: Starter, Studio and Enterprise
- Used by your in-house designers or engineers
- Starter covers the core workflow: CBCT/DICOM processing, implant planning with full control of depth/angulation/prosthetic positioning, restoration-driven workflows, surgical guide design, and unlimited export of manufacturing-ready STL and restoration models
- Studio adds AI segmentation of CBCT volumes, zygomatic implant planning, and bone reduction guides
- Enterprise adds prosthesis guides, and combined guides for partially edentulous and full-arch cases
GuideMia Implant Master is FDA 510(k) cleared (K121466) and built under an ISO 13485 quality system.
B) Implant planning and review software (clinic / doctor side)
Cympha Studio (studio.cympha.com) lets clinicians open the implant plan in any browser, review positions, and adjust placement if needed. Studio is where the plan is viewed and edited — the approval itself is recorded in Cympha, on the case's Review and Approve page, so the clinician sees the files and the lab slip before signing off. Designed to be simple and fast, with no installation required on the doctor's side.
C) Case Management, Sharing & Communication
Cympha (app.cympha.com) is the platform where you run the design service. One workflow handles:
- Case submission portals for doctors
- File upload (CBCT, scans, photos)
- Status tracking, case chat, and an append-only case event log spanning 22 event types — every status change, every file added or removed, every label, comment and approval, in order and permanently
- A fixed role catalogue: each person picks one role when they sign up and it does not change afterwards; lab staff join by requesting the lab manager's approval, and the manager holds the roster
- Read-only colleague sharing by email, for anyone who only needs to look at the case
- Per-case billing and invoicing
Cympha Studio (studio.cympha.com) is the browser-based 3D viewer for sharing plans with clinicians and patients — interactive 3D model viewing, snapshot/bookmark/annotation, 4D playback for orthodontics and multi-stage visualization for implants, no installation required on the recipient's side.
D) Manufacturing Capability
In-house 3D printing or an external printing partner. GuideMia exports validated STL files and master models for surgical guides, bone-reduction guides, verification jigs, and restoration-driven models.
E) Training, Support, and Services
- GuideMia Training: onboarding for designers and clinicians
- Workflow consulting: help labs define internal QA processes
- Technical support: case troubleshooting and software assistance
- Optional clinical review services: cases can be reviewed by qualified clinicians if required
2. What Types of Surgical Guide Cases You Can Offer
GuideMia supports a wide range of implant guide scenarios:
- Single-unit implants
- Multiple-unit implants
- Tooth extraction with immediate implant
- Full-arch cases
- Bone-reduction cases (Implant Master Studio tier or above)
- Zygoma implants (Implant Master Studio tier or above)
Start Simple First
When launching your service, it is strongly recommended to begin with single-unit and simple multi-unit cases, build internal QA processes, then gradually expand into full-arch, bone-reduction, and zygoma workflows. This reduces risk, improves turnaround time, and allows your team to standardize quality.
3. Who uses which software: lab vs. clinic
- Lab side. GuideMia Implant Master is used for case analysis, implant positioning, guide design, and manufacturing preparation.
- Clinic / doctor side. Cympha Studio is used for reviewing plans in the browser and requesting modifications; the final sign-off is given in Cympha, on the case's Review and Approve page.
This role separation keeps design efficient while ensuring clinical responsibility remains with the doctor.
4. How Cases Enter the System
Cases enter through Cympha: doctors submit through their case portal at app.cympha.com, upload CBCT, intraoral scans, photos and prescriptions, and the lab tracks status, communication and approvals against a permanent archive. For 3D plan review and patient-facing visualization, share a Cympha Studio link — no installation needed by the recipient.
5. The Surgical Guide Design Workflow
Step 1 — Case Submission
Doctor submits the case through Cympha. Data includes CBCT (DICOM), intraoral scans (STL), clinical photos, prescription/instructions.
Step 2 — Design in Implant Master
Lab designer imports CBCT and scan data, performs anatomical segmentation, implant planning, guide design, and prepares the plan for review.
6. Three Tiers of Case Review & Collaboration
GuideMia supports three levels of review, allowing you to balance cost, speed, and detail.
Tier 1 — Report-Based Review
Shared: PDF or web report, screenshots, short review videos. Use case: simple cases, fast approval, initial communication. Benefits: very lightweight, easy for doctors to review.
Tier 2 — 3D Composite Review
Shared: a single 3D model containing bone structure, teeth, CT cross-sections, and planned implants. Delivered as a Cympha Studio link the doctor opens in any browser — full spatial understanding, visual validation of implant position, no install on the clinician's side.
Tier 3 — Full Project File Review
Shared: complete GuideMia project file containing all segmentation, planning data, and design history. Useful when full editable detail is required; exchanged through Cympha file sharing.
GuideMia's approach: prefer Tier 1 and Tier 2 review for speed, and reserve full project files for cases that genuinely need them — Cympha Studio links replace the bulk of large-file exchange.
7. Approval and Manufacturing
- Step 3 — Clinical approval. Doctor reviews the plan in Cympha Studio, then approves or requests changes on the case's Review and Approve page in Cympha; every decision lands in the case event log.
- Step 4 — Finalization. Lab finalizes guide design in Implant Master, exports manufacturing files.
- Step 5 — Production. Print in-house or send to external manufacturer; deliver guide to clinic.
8. Operating Cympha and Cympha Studio Together
- Cympha is your operational system of record — formal case submission, status tracking, multi-client management, audit trails and archives, billing.
- Cympha Studio is your sharing surface — fast 3D plan review, patient-facing visualization, lightweight links you can send to any clinician.
Used together, they replace separate portals, file transfer tools, viewers and chat threads with one workflow.
9. Best Practices for New Labs
- Start with simple cases (single-unit, straightforward multi-unit)
- Standardize review checklists, report formats, and approval criteria
- Introduce Tier 2 and Tier 3 workflows gradually
- Prefer Cympha Studio links over full project file exchange whenever possible
- Keep design and clinical responsibilities clearly separated
